A 26-year-old man presented to the emergency department complaining of right chest pain and dyspnea just after a motorcycle accident. He was drunk but was still alert. A right anterior chest bruise without an open wound was noticed on physical examination, and the breath sounds over the right chest region were mildly decreased. A chest radiograph was taken immediately and revealed a large paramediastinum cavitary lesion (Figure 1). The patient was later intubated because of progressive hypoxemia. Computed tomography of the chest was performed to determine the extent of the lung contusion (Figure 2).
